Moon & Meadow Rescue uses the information you provide only for adoption screening, matching, and, if approved, preparing the final adoption agreement and providing follow-up support. Access is limited to our rescue team and we do not sell your data. Unsuccessful applications are kept for 6 months and successful adoption records are kept for 5 years. You can request access, correction, or deletion by contacting rescue@moonandmeadow.org.
If your application is approved, the dog will first be placed with you on a two week trial basis.
During the trial period:
Ownership
The dog remains the legal responsibility and ownership of Moon & Meadow Rescue until the trial period is completed and the final adoption agreement is signed.
Care
You agree to provide safe, appropriate daily care including feeding, exercise, supervision, and a calm settling period. You agree not to rehome, transfer, or leave the dog with another person without written approval from Moon & Meadow Rescue.
Vet care and communication
Moon & Meadow Rescue covers veterinary costs during the trial period. You must inform us immediately about any health concerns, injuries, behavioural issues, accidents, or incidents. You agree not to book non urgent vet treatment without approval. In an emergency, you may seek immediate veterinary help, but you must contact us as soon as possible.
Guidance
You agree to follow our guidance on decompression, routine, introductions, and training to support the dog’s adjustment.
End of trial
At the end of the two weeks, you will confirm either:
A. You wish to proceed with adoption and we arrange the final adoption agreement and transfer of ownership.
B. You do not wish to proceed with adoption.
If you do not proceed with adoption
You agree to keep the dog safe and cared for until Moon & Meadow Rescue collects the dog. We request up to one week maximum to arrange collection or alternative placement, unless there is an urgent welfare or safety issue requiring immediate removal.
Please complete this application carefully and honestly. If a question does not apply, write “N/A”. Incomplete or dishonest applications may result in disqualification. Submitting this application does not guarantee approval or adoption.
